This compatibility macro has been removed from FFmpeg long ago.

libavutil's macro definition is a compound literal, which is illegal
in C++.  Fixes yet another -Wpedantic warning.

The former is deprecated by C++14. The standard says they are the same:

The header defines all types and macros the same as the C standard library
header<stdint.h>.

This eliminates some overhead, because the compiler doesn't need to
consider these functions throwing.

Use the duration of the stream we're actually decoding - not the
"global" attribute AVFormatContext::duration which may differ.

This is the version present in Ubuntu Oneiric, the oldest distribution
with gcc 4.6.  Debian Squeeze is off target, because it has gcc 4.4,
which is unable to compile MPD anyway.

This commit drops all API compatibility hacks for older versions.

Pass a callback table to scan_file() and scan_stream(), instead of
returning a tag object.

Clean up the process before exiting.

This updates the copyright header to all be the same, which is
pretty much an update of where to mail request for a copy of the GPL
and the years of the MPD project. This also puts all committers under
'The Music Player Project' umbrella. These entries should go
individually in the AUTHORS file, for consistancy.

The signal_check library went out of order when we started using the
GLib main loop.  Convert the SIGHUP handler to use event_pipe instead.
