From the feature request: "I generally like to have crossfade on, but when it happens during such short tracks (e.g. 20 seconds or less) it doesn't really sound good as those tracks are not really meant to be crossfaded and intended to act as a bridge on their own." Sounds reasonable. This commit doesn't add an option, but hard-codes the limit to 20 seconds. If it turns out that users want to have it configurable, we can still add the option. This updates the copyright header to all be the same, which is pretty much an update of where to mail request for a copy of the GPL and the years of the MPD project. This also puts all committers under 'The Music Player Project' umbrella. These entries should go individually in the AUTHORS file, for consistancy.

The crossfading code shouldn't depend on the audio output code. Pass the current audio format to cross_fade_calc() and let it compare directly, instead of using isCurrentAudioFormat().

No CamelCase in the file name. The output_buffer struct is going to be renamed to music_pipe. There are so many buffer levels in MPD, and calling this one "output buffer" is wrong, because it's not the last buffer before the music reaches the output devices.

"LOG_H" is a macro which is also used by ffmpeg/log.h. This is ffmpeg's fault, because short macros should be reserved for applications, but since it's always a good idea to choose prefixed macro names, even for applications, we are going to do that in MPD.

decode.c should be a lot smaller; start by moving all code which handles cross-fading to crossfade.c. Also includes camelCase conversion.

Include only headers which are really required. This speeds up compilation and helps detect cross-layer accesses.
